Pharmacists oversee and manage the standard dispensing responsibilities for our community pharmacies. Additionally, the pharmacists provide a variety of clinical services to our patients including consultations, over-the-counter recommendations, screening for appropriate vaccinations and administration.

Continuous Glucose Monitoring

Our CGM devices measure a diabetic patient’s glucose levels throughout the day and night while providing interpretations for the practices and better outcomes for high-risk patients.

Transition Care Management

Transition Care Management

Pharmacists aid primary care providers to ensure there are no gaps in the patient’s care after discharge. Our team assists with medical decision-making during the transition period through medication reconciliation, patient counseling, and enrolling patients in additional services to prevent hospitalizations.

Remote Patient Monitoring

Remote Patient Monitoring

Through our digital technology, we are able to communicate between patients and providers while closely monitoring patients with complex disease states. We provide patients with programmed devices to monitor physiological parameters such as weight, blood pressure, blood glucose throughout the day while daily recordings or programmed alerts are transmitted through secure data to our clinicians. Pharmacists review this data in order to make recommendations to primary care providers and aid patients with medication compliance and lifestyle changes.

Our chronic care management focuses on value-based care plans, preventing complications and hospitalizations while improving quality scores. Pharmacists work with nursing staff to address patient concerns with providers and resolve barriers to care. 

Diabetes Education

Diabetes Education

Our diabetes education programs provide education, resources and diet plans to patients looking to manage their diabetes. Pharmacists provide traditional patient counseling to ensure proper administration of antidiabetic medications. By considering specific patient factors such as comorbid conditions, health literacy, insurance formularies, and more, pharmacists are able to help create in-depth and individualized plans for our diabetic patients. 

Principal Care Management

Principal Care Management

Principal care management helps provide reimbursement for managing a patient’s care for a single high-risk disease or complex chronic condition. Patients qualify with a complex chronic condition lasting at least three months, if the condition requires development or revision of a disease-specific care plan, the condition is of sufficient severity to place a patient at risk of hospitalization, or the condition requires frequent adjustments in the medication regimen and/or is unusually complex due to comorbidities.

Practice Agreement

Our pharmacists work hand-in-hand with providers to manage chronic disease states and improve patient outcomes. Through collaborative practice agreements, pharmacists adjust medications and optimize therapy based on patient-specific factors. Pharmacists are able to see patients for “Incident-To” visits between a patient’s regularly scheduled office visits with their primary care provider. These Incident-To visits allow for more attentive monitoring of patients while alleviating some of the time constraints of the provider. This expansion of the role of pharmacists further increases patient access to quality healthcare.

Pill Box Program

Our community pharmacies offer compliance packaging to help patients with adherence to complex medication regimens. Pharmacists complete full patient workups for each patient enrolled in our Pill Box program. This includes reviewing labs, drug interaction check, and sending refills through collaborative practice agreements.
